When an individual or family is considering homecare, it can feel overwhelming and sometimes even confusing. This process involves several steps and is undertaken with a great deal of care and consideration. The assessment process helps ensure that the person in need receives quality, person-center care that meets their specific needs. Let’s take a look at how this process works.


The Homecare Assessment Process Explained:

The homecare assessment process begins when you contact us. During this initial phone call or email, we will ask questions about your loved ones health and living situation so that we can determine if homecare is necessary or if other options would be more appropriate (we will always guide you to whatever is best for you). After the initial call or email correspondence is complete, we schedule a home assessment at your earliest convenience. There are no wait times on our end, we are able to see you the next day if you would like.


At this point, our client care manager will visit the you and your loved one in their home to evaluate their physical and mental condition as well as the safety of their living environment. This assessment includes a discussion with the client and their families regarding their current situation, what the needs are, any concerns that the family may have regarding the safety and care for their loved ones. The client care manager has a series of questions that they will go through ranging from general personal information, diagnosis, dietary considerations and activities of interest.

After completing the assessment, the client care manager will create an individualized care plan for providing care for the client based on their needs and abilities. This plan may include services such as personal care assistance, medication reminders, help with grocery shopping or meal preparation, light housekeeping duties, companionship and more. It is important to note that all of these services are tailored specifically to each individual client’s needs so that they can receive quality, person-centered care without sacrificing their independence or privacy.
