At Remember When Healthcare, we understand that managing health at home can be challenging, especially for individuals living with chronic conditions, recovering from surgery, or requiring specialized medical attention. Our private nursing care at home is designed to provide professional, compassionate care right in the comfort of your home, ensuring that your unique health needs are met with the highest level of care.
Our team of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) bring the expertise and experience to handle a variety of medical situations. Whether you or a loved one requires consistent nurse care at home for a chronic condition, acute treatment, post-op recovery or palliative support, we are here to help!
Our Private RN Services & LPN Services
We offer personalized private nursing care at home, promoting holistic health and healing, managing symptoms, and supporting your overall well-being. Here’s an overview of the nurse at home services we provide:
Wound Care and Dressing Changes: Whether you're recovering from surgery, managing a chronic wound, or require dressing changes, our experienced nurses are here to help. We follow best practice guidelines to ensure proper wound healing, minimize discomfort and reduce the risk of infection. Our goal is to help speed up recovery so you can get back to feeling your best.
Medication Administration and Management: Managing medication can be overwhelming, especially when juggling multiple prescriptions. Our nurses can assist with administering oral, intravenous (IV) and subcutaneous medications, ensuring the correct dosages at the right times, and offering guidance on managing side effects. We can complete medication reviews and provide education and recommendations to ensure your treatment plan is followed precisely.
Chronic Disease Support: Chronic conditions such as diabetes, hypertension, or COPD need ongoing care and attention. Our team of experienced nurses are here to support you in a way that reflects your lifestyle and personal preferences. We monitor symptoms, vital signs, provide education and promote independence. We want to ensure you have the knowledge and support you need to be comfortable and in control of your health.
Palliative and Hospice Care: Facing a severe illness can be overwhelming, but we're here to offer comfort and support. Our palliative and hospice care focuses on easing pain, providing emotional support, and helping families navigate this difficult time with dignity and peace.
Health Assessments and Vital Sign Monitoring: Regular check-ups are key to staying healthy. Our nurses that come to your house can monitor blood pressure, heart rate, glucose, respirations and oxygen levels to catch any issues early and adjust your care as needed, keeping you on the path to better health.
Post-Operative Support: The recovery period after surgery is a vulnerable time. Our nurses are able to provide assistance with wound care, pain management, mobility, and physiotherapy-approved exercises, fostering recovery and healing in the comfort of your own space, and helping you to get home faster.
Mobility and Safety Assistance: Maintaining safety and mobility at home is essential for preserving your independence. Our nurses are able to complete transfer and mobility assessments, fall risk assessments and provide recommendations for household modifications, ensuring a safe environment at all times.
Blood Collection: Getting to a clinic for a blood test can be difficult if you have mobility challenges or transportation issues, but we’re here to help. A member of our nursing team will come to your home, collect the sample and ensure it’s delivered to the lab, with results sent directly to your physician.
