About Remember When Homecare
Remember When Homecare is a family-owned and operated company established in 2021. Our personal experiences working in the industry as well as caring for our loved ones presented us with difficulties finding a person-centered, professional homecare provider in Nova Scotia. We wanted to build a homecare company where the client's individuality chooses the course of services provided. We take into account every client's life experiences, age, gender, culture, heritage, language, beliefs, and identity. Remember When Homecare’s clients are acknowledged as the experts in their own life with an emphasis on their abilities, while offering a helping hand with any assistance that may be required. At Remember When Homecare, we want to build and uphold a caring, compassionate, and supportive environment for our clients and their families.
1. Initial Outreach
You can reach out to us via telephone, email or on our social media accounts. As a new client, you will speak to a member of our nursing team, either Steph or Jenna via telephone to address any initial questions and arrange a suitable date and time for a free, in-home assessment.

After speaking with member of our nursing team, we will schedule a time to meet with you and/or your loved one in your home.
During this visit, we will discuss:

This is a great opportunity for the primary client or other family members to ask questions or address concerns. Any and all members of your support system are welcome to be a part of this process as we work together to find the best homecare solutions for you.
At RWHC, we aim to foster strong connections between clients, families and caregivers. We complete a care plan that clients, family members and caregivers are able to access on our Caresmartz360 online family portal.
Clients and any members of their care team whom they would like to have access to this information will be able to:

We pride ourselves on transparency and want you to have visibility and oversight on your homecare services.
4. Schedule Creation
During our initial assessment, we will collaborate with you to implement a schedule that meets your individual needs. We have modernized the way that homecare services are delivered by completing schedules 6-8 weeks in advance so that you do not have to worry about coverage.
A member of our team will reach out to you to update you on any scheduling changes, and if you have any changes that need to be made, you can always reach out to our Care Scheduling Team via phone or email.

A member of the care scheduling team will reach out to the caregiver before the first scheduled visit to ensure clarity of care plan.
Homecare services will commence within 1-2 weeks after the initial assessment date, or sooner if this has been previously agreed upon. We will contact you prior to the first scheduled visit to verify the date and time and provide you with the name of the caregiver coming, and ensure you are able to access our online scheduling system.
A member of our nursing team will reach out to you to ensure you are satisfied following the first scheduled visit with your assigned caregiver and address any questions or concerns you may have.

7. Care Plan Updates
Depending on your care needs, a member of our nursing team will be in contact a minimum of once monthly to update your care plan and ensure your satisfaction with our services. At RWHC, we understand that our clients are the experts of their own lives. Your care plan is your choice, and our role is to support you in living your life according to your personal preferences.
You can reach out to us at any time if you would like to make any changes, and if physical needs change significantly, a member of our nursing team, Steph or Jenna will reach out to schedule an in-home care plan re-assessment. We also like to pop by for a visit during the holiday season!

We are concerned that our loved one is lonely, can you help with that?Yes, one of the services that we offer is companion care. Our friendly caregivers will visit clients and provide companionship. We engage clients to brighten their days, whether they are there just to listen and converse or play cards & board games, your loved ones will be taken care of as though they are our family.

How do I get started?That's easy. Contact us by phone or email and we will set up a free home assessment done by one of our care managers at your home. We will talk about your needs, how best we can meet them, then we will select a team of caregivers that are qualified to meet your needs and we will set up the service based on your schedule.

How long is the wait time for services?Our caregivers are ready to start when you decide you would like to start the service.

We are the primary caregivers to our loved one and need someone to come in to give us a break so we can run errands, is this something you do?Yes, we offer Respite Care for family caregivers. Those caring full-time or part-time for a loved one are at risk of suffering from caregiver burnout. That’s why it’s critical to get regular breaks. We are ready to step in and provide respite for you on a short-term or long-term basis.

How much do your services cost?Our fees are based on an hourly rate. We offer a complementary assessment where we assess your needs and create a care plan together. At that time we will provide you with an estimate of costs, and you are under no obligation to follow through.

Is there a minimum amount of time required for each visit?The minimum time requirement for our homecare services is 4-6 hours per visit.

How do you invoice for the services?We issue weekly or bi-weekly invoices for services rendered, this depends on your preference. We will send you an invoice and you can pay via e-transfer, cheque or credit card.

I am nervous about having people I don’t know in my house, are you caregivers bonded?All of our caregivers undergo a thorough background check and a vulnerable sector check before being hired by us. All have experience in the field of caregiving.

Do your caregivers have any professional training?Many of our caregivers are CCA’s and PSW’s, and others are companion care givers who receive training from Remember When Homecare.

I don’t want someone different each time, can you accommodate that?We match our caregivers with our clients based on personalities and suitability. We try to schedule the same caregivers each time the best we can to accommodate the bonds that are made.
